Michigan/CreativeCurriculum.net Eligibility Verification Form

In order to qualify for the special pricing for CreativeCurriculum.net or OSEPReports.net, districts and qualified programs need to submit an online form to obtain MDE eligibility verification for special pricing. Only one application per organization is required. The primary authorized administrator or his/her designee should complete this form to begin the process. Complete the Eligibility Verification form below.

Once your application has been verified, an Account Manager from Teaching Strategies will contact you to complete the subscription process. Please be aware that this form is not your subscription, and that you will need to complete additional steps to finish the process.
